Patients can conveniently and securely request a copy of their medical records from the hospital. Please ensure all information is completed accurately so that our staff can verify and prepare the documents promptly according to the prescribed procedures.

Request by the Patient

Required documents include:

• Patient’s national ID card with a certified true copy
• Proof of payment

Request by an Authorized Representative

Required documents include:

• Power of attorney from the patient
• Certified true copy of the patient’s national ID card, signed by the patient only
• Certified true copy of the authorized representative’s national ID card
• Proof of payment

Request by a Legal Representative for Minors or Deceased Patients

Required documents include:

• Certified true copy of the patient’s national ID card or birth certificate
• Certified true copy of the legal representative’s national ID card
• Proof of payment
• Documents demonstrating relationship, such as:
  • For children: Certified true copy of house registration
  • For parents: Certified true copy of house registration
  • For spouse: Certified true copy of marriage certificate
