Inguinal hernia

What is inguinal hernia?

An inguinal hernia is a condition when internal part of your body such as intestine pushes through a weak spot of your muscle. This can either result in pain when lifting heavy objects and bending over or no pain at all. Inguinal hernia is a condition that does not improve on its own and may lead to life-threatening complications later on.

Causes

  • ◼️ A preexisting weak spot in the abdominal wall
  • ◼️ Age
  • ◼️ Birth defect
  • ◼️ Chronic coughing or sneezing
  • ◼️ Pregnancy
  • ◼️ Previous abdominal surgery
  • ◼️ Putting pressure on abdomen such as straining on toilet from constipation and lifting heavy objects

Symptoms

Inguinal hernia signs and symptoms include:

  • ◼️ Swelling or lump in your groin, can be seen when coughing or lifting heavy object
  • ◼️ Appears as enlarged scrotum
  • ◼️ Pain
  • ◼️ Weakness or pressure in your groin
  • ◼️ In newborns, sometimes visible only when crying, coughing or straining during bowel movement
  • ◼️ Newborns may be irritable and have lesser appetite than usual

Serious Complications

Complications from inguinal hernia can be life-threatening if left untreated. If the hernia cannot be pushed back in, it could be trapped in the abdominal wall and can become obstructed or strangulated.

  • ◼️ Obstruction: a section of bowel becomes stuck in inguinal canal. Symptoms include nausea, vomiting, stomach pain, and painful lump.
  • ◼️ Strangulation: a section of bowel becomes trapped and blood supply cannot flow. Emergency surgery needs to be done within hours to release the trapped tissue.

Treatment

Inguinal hernia can be repaired with surgery and is recommended if you have pain, severe or persistent symptoms, or if any serious complications develop. There are two main types of inguinal hernia repair:

  • ◼️ Open surgery: a cut where surgeons can push the lump back into the abdomen
  • ◼️ Laparoscopic surgery: a less invasive technique requiring experienced and skilled surgeons for the repair

When to see a doctor

If you see the hernia bulge turning red, purple, or dark, you may be at risk for inguinal hernia strangulation which is a life-threatening complication. In this case, immediately contact emergency room. However, if you see a lump suspected to be inguinal hernia with or without symptoms such as pain, make an appointment with surgery center for diagnosis. If inguinal hernia is confirmed, a repair can be made before complications set in. In some cases, follow-up monitoring is recommended for mild cases.

 

Appendicitis

What is appendicitis?

Appendicitis is an inflammation of the appendix, which is on the lower right side of your abdomen and causes pain. In most people, pain begins around the navel and then moves. As inflammation worsens, appendicitis pain typically increases and eventually becomes severe.

Causes

The likely cause is infection in the appendix causing inflammation. Left untreated, the appendix can rupture which is life-threatening and requires surgery to remove the appendix and clean your abdominal cavity.

Symptoms

Signs and symptoms of appendicitis may include:

  • ◼️ Sudden pain that begins around your navel and often shifts to your lower right abdomen
  • ◼️ Sudden pain that begins on the right side of the lower abdomen
  • ◼️ Pressing may worsen the pain
  • ◼️ Nausea and vomiting
  • ◼️ Loss of appetite
  • ◼️ Constipation or diarrhea
  • ◼️ High temperature

Treatment

Treatment usually involves surgery and antibiotics treatment.

  • ◼️ Open surgery: For cases of ruptured appendix, infection spread, and abscess is present, your surgeon will need to clean the abdominal cavity and may need to place a tube to drain abscess and prescribe antibiotics to control infection before removing appendix later
  • ◼️ Laparoscopic surgery: Lesser invasive version of surgery with small incision. Good for cases without complications needing open surgery.

If you feel pain which may correspond to appendicitis, contact your doctor or emergency room to receive treatment as earliest as possible.

 

Gallstones

What is Gallstones?

Gallstones are small stones and also called (Cholelithiasis). Most gallstones are made up of cholesterol. This originated from gallbladder which stores and releases bile carrying cholesterol and bilirubin potentially forming gallstones.

Risk Factors

  • ◼️ Family history
  • ◼️ Woman
  • ◼️ Obesity
  • ◼️ High fat and cholesterol diet and low in fiber
  • ◼️ Pregnancy
  • ◼️ Diabetes
  • ◼️ Losing weight quickly in short time
  • ◼️ Blood disorders such as hemolytic anemia
  • ◼️ Lack of exercise and physical activity

Symptoms

Gallstones often do not cause any apparent symptoms and usually found accidentally with health screening except when gallstones were trapped in a duct, it could trigger result in acute symptoms:

  • ◼️ Acute and intense pain in abdomen
  • ◼️ Back pain
  • ◼️ Nausea or vomiting

Treatment

Patients without symptoms may not need to treat at all. The patient will then be advised to be vigilant on any signs and symptoms which may associate with gallbladder stones and treatment will be based on symptoms:

  • ◼️ Surgery (Laparoscopic Cholescystectomy) to remove gallbladder stones with low risk of complications
  • ◼️ Open Cholecystectomy makes a larger cut if indicated by your doctor
  • ◼️ Medications to dissolve gallbladder stones

When to see a doctor

If you have any symptoms of acute pain that bothers you, make an appointment with your doctor to examine the cause. If you have serious complications such as intense pain in your belly, skin and eyes turning yellow (jaundice), or high fever, seek medical care immediately.

 

Hemorrhoid

What is Hemorrhoid?

Hemorrhoids are swollen vein inside the rectum or outside anus which can cause pain, itching, and bleeding. There are several types of hemorrhoids: External (Underneath the skin of anus), Internal (Inside the rectum), and Prolapsed (Stretch and bulge out of anus).

Risk factors

Symptoms

Treatment

You can start with self-treatment for mild symptoms

  • ◼️ Eating high fiber food
  • ◼️ Use hemorrhoid cream or suppository
  • ◼️ Soak regularly in warm or sitz bath
  • ◼️ Take pain relievers

When to see a doctor?

You can seek medical attention if there is bleeding with bowel movement or you have thrombosed hemorrhoids that does not improve.
